This is one of the most intriguing Medic builds we have tested in Fallout 76. It gives amazing team utility via direct healing, V.A.T.S. healing with the Medic's Legendary weapon, and on top of being useful as a medic, Heavy Guns with high Accuracy and Critical Strike Chance have terrific DPS on their own. In our opinion, healing, the Suppressor, and the One Gun Army Perks make this the best end-game support characters in Fallout 76.
Below you will find all the recommended Perks for each of the SPECIAL attributes. We have also included several item recommendations for this build, and listed the most useful mutations and Chems that you can use. Just keep in mind that this is an end-game build, and it will perform correctly with level 50 or above, all the necessary Perks, and decent Medic's Heavy Gun.

SPECIAL Attributes and Perk Cards
STRENGTH [9 POINTS] |
Gain +60 damage resistance while firing a heavy gun Heavy guns reload 30% faster The weight of all Chems (including Stimpaks) are reduced by 90% |
In the current state of Fallout 76, Strength is one of the most important attributes in this build. It increases your maximum Carry Weight, and reduces the weight of your Chems and Stimpacks, allowing you to actually play the character without the necessity to constantly visit your Camp |
PERCEPTION [10 POINTS] |
You can view a target`s specific damage resistance in V.A.T.S. V.A.T.S. now targets limbs. Focus fire to gain high accuracy and damage per shot Reap twice as much when harvesting flora You now gain +40 Energy Resistance |
Perception increases your Accuracy in V.A.T.S. which is essential in this build, that's why we want to spend about 10 Points here |
ENDURANCE [1 POINT] |
You gain increased benefit from being Well Fed or Well Hydrated |
Not spending any of the SPECIAL points here may not be a popular idea, but we tend to believe these can be spent better elsewhere. Unless you suffer high damage spike, you should be able to heal all the damage with high Perception and Luck attributes |
CHARISMA [12 POINTS] |
Gain 12 Damage & Energy Resist (max 36) for each teammate, excluding you Gain 2 Charisma for each teammate, excluding yourself Your stimpaks now also heal nearby teammates for their full strength Reduce your target's damage output by 30% for 2 seconds after you attack |
Charisma is the primary attribute of every team player, and this build is mostly suited for supporting your teammates. While it still has solo potential, there are much better builds out there for that |
INTELLIGENCE [6 POINTS] |
In Power Armor, heavy guns gain excellent accuracy and ignore 45% armor Stimpaks restore 45% more lost health |
Intelligence grants access to two extremely important Perks for this build. The choice of First Aid is self-explanatory, while Stabilized provides the very high DPS bonus and Accuracy for more consistent Crits. |
AGILITY [3 POINTS] |
Action Points regenerate 45% faster |
Action Points regeneration is very important when you want to constantly use V.A.T.S. for extra damage and healing. Action Boy/Girl is a must-have |
LUCK [15 POINTS] |
V.A.T.S. criticals now do +40% damage Critical Hits now only consume 55% of your critical meter Each hit in V.A.T.S. has an excellent chance to fill your Critical meter Heavy guns gain a 12% stagger chance and a 12% chance to cripple a limb Any kill in V.A.T.S. has a 15% chance to refill your Critical Meter |
Luck is one of the most important SPECIAL attributes in this build. Luck grants access to the core Perks used in this build, which are related to high Crit. Chance, damage, and team utility. Luck will passively help you sustain high DPS and V.A.T.S. healing |
Equipment
ARMOR: Power Armor is a must-have in this build. Power Armor grants unparalleled Damage Resistance, Energy Resistance, increased Carry Weight and can have numerous useful modifications WEAPON: Medic's Heavy Gun is required for this build's core mechanic to work. Gatling Gun, Gatling Laster, .50 cal machine gun, and Minigun all have decent DPS. Just make sure they are level 45/50 if you want to do some damage, instead of just healing your teammates CHEMS: Orange Mentats provide high bonuses to your V.A.T.S. Accuracy and Perception and Daytripper increases your Luck (hence your overall chance to Critically Hit as well). Also, there is a bunch of food and drinks that can be used to increase your Luck temporarily; all of which works great in this build. |
Mutations
Certain Mutations are extremely powerful for this build. They are, however, quite hard to get, and will require you to have the Starched Genes Perk which will prevent them from getting healed by Rad-away.
